Job summary

Spurwink is seeking a Care Coordinator for the Biddeford BHH team. You will coordinate with multiple providers to address home, school, medical, and community needs for children and their families in York County.

The role is 40 hours per week, with a pay rate of $23.00–$27.00 per hour and a $2,500 sign-on bonus. Strong organizational and communication skills, plus solid computer proficiency, are essential for managing intake, assessments, and ongoing care coordination.

Qualifications

• BA in related field required.
• Strong organization and communication skills.
• Need savvy computer skills to manage multiple systems.

Responsibilities

• Complete intakes for new clients.
• Complete psychosocial assessments and other intake assessments.
• Complete weekly documentation needs, make referrals, and complete initial and 90-day treatment plans.
• Provide a minimum of monthly contact with each family and collaborate with providers.
