• Licensure or provisional licensure as a Social Worker, Clinical Social Worker, Independent Social Worker, or Associate in Social Work.

Responsabilidades

• Assess and reassess client levels of functioning; interview clients and family members; review reports to determine needed services.
• Develop and implement a case plan with intervention strategies and placement options within policy and budget constraints.
• Coordinate services with state agencies, community organizations, and private providers to meet case plan goals.
• Provide ongoing case management through home visits, monitor services, and reassess client needs; track progress and resources.
• Organize MDT meetings to plan protective services and coordinate with education, law enforcement, and health professionals.
• Document case activities and prepare reports per policies and regulations; ensure confidentiality.
• Provide crisis intervention and report neglect or abuse to authorities; follow safety protocols.
• May provide guidance and supervision to other workers and staff.
• Perform related duties as assigned.
