Job summary

Qualifications

• Active Nevada Social Work licensure required or licensure eligible.
• Experience with case management and family-centered services preferred.
• Ability to conduct home visits and coordinate services across agencies.

Responsibilities

• Develop and implement a case plan with intervention strategies to meet client needs and promote permanency.
• Perform home visits, monitor services, and reassess client progress and well-being.
• Prepare court reports and maintain case records according to policies and regulations.
• Locate and coordinate services from state agencies and private providers to support client goals.
• Provide crisis intervention and coordinate MDT meetings when needed.
• Assist in supervising and training non-professional staff as assigned.
