The Child Case Manager is responsible for providing high-quality, person-centered case management services from intake through discharge or transition. This role supports individuals with mental illness, intellectualdisabilities, or substance use disorders in achieving improved health, stability, and independence. Duties include coordinating services, maintaining documentation, and collaborating with a multidisciplinary team to ensure persons servedreceive timely, effective, and compassionate care.

Qualifications and Education Minimum Requirements
Applicant(s) must be qualified as a CMHP with 3 years experience with a bachelor's degree or 1 year experience with a master's degree in the examination, evaluation, and treatment of minors and their families. State license as an LMSW, LLMSW, LMFT, LLMFT, LBSW, LLBSW,LPC, LLPC, TLLP, LLP, LP, RN,or a degreein a human service field from an accredited school. Human service fields can include, but not limited to Human Services, Psychology, Sociology, Criminal Justice,or Social Work.
Must have a valid Michigan driver’s license, current automobile insurance, and the use of the insured vehicle during work hours.
Essential Conditions
Physical: Standing, support weight other than own, walking, stooping, kneeling, carrying, reaching, pushing, pulling, use of telephone, general good hearing, general good vision, speaking, physical intervention with clients, ability to travel, and ability to transport persons served.
Mental: Reading, writing, simple arithmetic, analyze data/report information, work under pressure, rapid decision making, create methodologies for solving client goals, implement recommendations, pleasant, congenial, calm, courteous, firm, and ability to maintain confidentiality. Direct activities of others to accomplish a goal and evaluate the performance of others.
Environmental: Community settings and persons servedhomes, exposure to public and possible aggressive clientele or poor health or hygiene, ability to work evenings and weekends, as necessary.
