Salary: $20 per hour

Qualifications:
  • Bachelor's degree in social work, psychology, or a related field
  • OR at least 3+ years of experience in a social service or healthcare-related setting
Main Function:

Service Coordination involves identifying, coordinating, and facilitating all necessary supportive services and community resources for elderly and/or disabled participants based on their individual needs. Service Coordinators will work from home as their primary office, with travel required to visit participants' homes a few times a week.

Responsibilities:
  • Attend and complete training and ongoing annual training (remote)
  • Manage an active caseload by monitoring and evaluating options and services to meet individual health needs
  • Complete person-centered assessments and assist members in obtaining HCBS services supporting independent living
  • Assist members in gaining access to LTSS and Medical Assistance services
  • Conduct monthly monitoring calls and develop or modify service plans at least annually or as needed
  • Maintain ongoing communication internally and with caseload
  • Inform members of their right to choose any willing and qualified provider for services on their plan
  • Participate in remediation activities
  • Manage a caseload of 70-80 participants
  • Maintain communication with providers
  • Ensure participants exercising participant-directed budget authority do not exceed approved service hours
  • Report all member incidents and allegations of neglect/abuse within 24 hours
  • Follow company policies and procedures for documentation, confidentiality, records, and employees' rights
  • Willingness and ability to work in the field and independently in a virtual setting

Additional Requirements: Reliable transportation is necessary. Bilingual skills are a plus but not required.
