Job summary

Astor Services in New York seeks a Care Manager to coordinate care for a caseload of children and families under the state's health homes program. You will conduct needs assessments, develop an Individual Plan of Care, and connect enrollees with Medicaid, SNAP, housing, and community supports.

You will document eligibility and services in the Medicaid Analytics Performance Portal and EHRs, participate in team meetings, advocate for clients, and help families navigate the health system to improve

Qualifications

• Bachelor's Degree is required.
• Care Management experience is required.

Responsibilities

• Completes initial and ongoing needs assessments to determine level of care management.
• Manages the overall care plan and coordinates services per acuity level.
• Conducts care planning meetings and coordinates with an interdisciplinary team.
• Documents eligibility and services in care management portals and EHRs.
• Advocates for clients and connects them with community resources and supports.
