Job summary

Abbott House in Bronx, NY is seeking a Care Manager to join our care coordination team. The role focuses on coordinating services, guiding enrollees and their guardians through the health care system, and ensuring access to required supports to prevent hospitalizations and maintain stability.

The Care Manager conducts needs assessments, develops and monitors Individualized Plans of Care, advocates for clients, and collaborates with providers.

Qualifications

• Associate Degree with two years’ experience.
• Bachelor’s Degree with two years’ experience.
• Registered Nurse or Licensed Practical Nurse with two years’ experience.
• A Master’s Degree in related field (Licensed preferred).

Responsibilities

• Obtain care management enrollment consents from the individual or legal guardian.
• Complete initial and ongoing needs assessments (CANS) to determine level of care management.
• Responsible for the overall management of the patient’s Individualized Plan of Care.
• Coordinate the enrollee’s provision of services as per their acuity level.
• Support adherence to treatment recommendations.
• Monitor and evaluate a patient’s needs, including preventive and medical care.
• Meet client contact requirements and maintain regular visits.
• Maintain Care Management documentation in MAPP and EHRs.
• Advocate for clients within the agency and external providers.
• Promote wellness by linking enrollees to resources and services.
• Educate on chronic conditions, immunization, and preventative care.
• Help clients obtain public benefits such as Medicaid, SNAP, housing, and more.
• Communicate with families considering language and literacy needs.
• Conduct care planning meetings and coordinate comprehensive care.
• Identify community resources and manage referrals and follow-ups.
