Job summary

TAG MedStaffing is seeking a Children's Care Manager in Bronx, NY to support care coordination, health services, and community resources for children and families. This in-office role will manage a caseload, conduct assessments, and coordinate services across healthcare and social service providers throughout New York City.

The position emphasizes field-based client support, documentation, and collaboration with multidisciplinary teams, with Monday–Friday scheduling and strong communication

Qualifications

• Bachelor's degree in Health, Human Services, Social Work, Psychology, or a related field is required.
• 1+ years of care management, case management, healthcare coordination, child welfare, or human services experience is required.
• Knowledge of children's health services, behavioral health, foster care, community resources, or social service programs is required.
• Able to maintain electronic case records and learn NYS social services and care management systems.

Responsibilities

• Care Coordination: Coordinate healthcare, behavioral health, educational, and community-based services for children and families.
• Client Assessments: Complete health, developmental, and needs assessments and identify services.
• Case Management: Maintain regular client contact, monitor care plans, and track changes.
• Provider Coordination: Collaborate with caregivers, healthcare providers, and social service teams.
• Transitional Care: Support clients through hospitalizations, discharges, and transitions.
• Documentation: Maintain accurate case records and billing information in electronic systems.
