Job Scope

As a member of the Care Coordination team and under the supervision of the Program Supervisor, the Care Coordinator is responsible for addressing all member needs, providing care plan updates, and conducting outreach to members in between visits. Care Coordinators provide care coordination to NYC Medicaid beneficiaries with chronic health and/or behavioral health disorders using a Health Home service model. They advocate and support members, engage with community agencies/health care providers, and others on the member’s behalf to ensure access to services that increase wellness, self-management, and reduce emergency room visits and hospitalizations.

Essential Functions
  • Coordinate care for a caseload of 40-50 members.
  • Maintain monthly contact with all members of the caseload, with increased contact for newly enrolled and high-risk members.
  • Conduct member engagement activities, including face-to-face, mail, electronic, and telephone contact, upon handoff from the Outreach Team.
  • Establish and maintain effective communication with primary and specialty care providers, mental health and substance abuse providers, family, and other resources on behalf of members.
  • Maintain organized and timely documentation, records, statistics, and reports as per policy.
  • Conduct initial and periodic needs assessments, considering barriers, assets, member and family preferences, language, literacy, and cultural factors.
  • Assist with developing and implementing care plans, helping members understand and follow instructions, and tailoring communications to health literacy levels.
  • Record client progress with measurable goals from the care plan.
  • Assist members in accessing healthcare and social systems, including transportation, scheduling, and accompanying them to appointments.
  • Identify community resources, manage referrals, and coordinate services.
  • Support members in managing daily routines related to healthcare, considering their strengths and barriers.
  • Conduct outreach and engagement activities to ensure continuity of care, re-engaging members as needed.
  • Provide crisis intervention and follow-up.
  • Monitor member entitlements, insurance, and benefits to ensure they remain active.
  • Advocate for members in crisis situations.
  • Collaborate with professionals to evaluate health conditions and assess needs.
  • Manage wrap-around funds, metro cards, and checks for member purchases, obtaining necessary approvals.
Qualifications
  • Bachelor’s degree and at least 2 years of experience providing direct services to individuals with mental disabilities or connecting them to essential community services.
