The Director of Quality at Joseph P. Addabbo Family Health Center, Inc. will lead the design and oversight of the organization's Quality Improvement, population health, and performance management program to enhance patient outcomes and engagement.

You will work cross-functionally with clinical, analytics, and operations teams, ensure HRSA, NCQA PCMH, and UDS requirements are met, and drive data-driven improvements through QA/QI initiatives and regular reports.

Qualificações

• Bachelor’s degree in healthcare administration, public health, nursing, or related field is required.
• Master’s degree in public health, healthcare administration, or related field preferred.
• RN/NP with healthcare quality improvement experience will be considered in lieu of a master’s degree.
• Minimum 5 years of healthcare quality or performance improvement experience.
• Strong knowledge of HRSA, UDS, NCQA PCMH and related regulatory requirements.
• Proven ability to lead cross-functional teams and drive quality initiatives.

Responsabilidades

• Directly supervise the Quality Assurance Coordinators.
• Develop and update QA policies and procedures.
• Liaise with MCOs and participate in quality meetings.
• Proficiency with the Azara platform to support clinical workflows.
• Collaborate with the IPA to drive quality performance and value-based contracts.
• Utilize data stratification and risk adjustment tools to identify high-risk patients.
• Ensure compliance with HRSA PCMH standards.
• Coordinate quarterly provider peer reviews and QA/QI activities.
• Conduct on-site QA/QI implementation visits and huddles.
• Aggregate quality data and prepare narrative reports with analytics.
