Job description

The New Jewish Home Certified Home Health Agency (CHHA) provides home based care and rehabilitation therapy for patients recovering from an acute episode or other adverse health event. The New Jewish Home has long experience delivering rehabilitation and home care services. Our goal is to help each patient get well, stay well and return to health and independence as quickly as possible.

The CHHA is currently seeking a part time Physical Therapist to join their team! The selected candidate will receive an incentive bonus of $3000 prorated for part time employees.

Job Summary

Responsible for providing and coordinating multidisciplinary health services for patients in the community. Performs the role of Physical Therapist and works with the physician and other team members in the implementation of plan of care.

Duties and Responsibilities

• Collaborates with physicians, patients and their families, nurses, therapists, social workers and other members of the patient care team to develop and implement patient specific care and treatment plans.
• Utilizes varied solutions to educate and promote the health and independence of each patient.
• Assess, monitor, document and report progress of patients’ health and condition using required documents, reports and established timelines. Provides supervision and evaluation of aides and physical therapy assistants.
• Observes signs and symptoms of patients, reports to the physician and/or other appropriate staff as often as needed or as change in the patients’ condition requires.
• Completes OASIS assessments timely, accurately and according to OASIS user’s manual definitions. Completes all documentation, patient health status reports as required and forwards appropriately.
• Supervises occupational therapy assistant according to state regulations and agency policy.
• Assists with the coordination of the total plan of care and maintains continuity of patient care by liaising with other appropriate staff assigned to the same patients. Participates in team/case conferences. Initiates team/care conferences for complex and/or multiple disciplinary patients when required to ensure coordination of care.
