Job summary

The New Jewish Home is seeking Fee for Service Physical Therapists to join our Certified Home Health Agency team in New York. Responsibilities include coordinating multidisciplinary health services and carrying out plan of care with physicians and the care team.

You will assess and document patient progress, educate patients and families, and supervise therapy aides to ensure high-quality home-based rehabilitation. A NY PT license and degree are required, with 1 year of experience preferred.

Qualifications

• Must possess a current, unencumbered license to practice as a Physical Therapist in the State of New York.
• Master’s degree or Doctorate degree in Physical Therapy.
• One year of Physical Therapy experience preferred.
• Ability to communicate effectively both orally and in writing.

Responsibilities

• Collaborates with physicians, patients and families, nurses, therapists and other care team members to develop and implement patient-specific care plans.
• Education and promote the health and independence of each patient.
• Assess, monitor, document and report progress using required forms and timelines; supervise aides and PT assistants.
• Observe patient signs and symptoms and report to the physician or staff as conditions change.
• Complete OASIS assessments accurately and timely and document patient status as required.
• Coordinate the total plan of care and maintain continuity of patient care through team conferences.

Job Summary

Responsible for providing and coordinating multidisciplinary health services for patients in the community. Works with the physician and other team members in implementation of plan of care.

Duties And Responsibilities

• Collaborates with physicians, patients and their families, nurses, therapists, social workers and other members of the patient care team to develop and implement patient specific care and treatment plans.
• Utilizes varied solutions to educate and promote the health and independence of each patient.
• Assess, monitor, document and report progress of patients’ health and condition using required documents, reports and established timelines. Provides supervision and evaluation of aides and physical therapy assistants.
• Observes signs and symptoms of patients, reports to the physician and/or other appropriate staff as often as needed or as change in the patients’ condition requires.
• Completes OASIS assessments timely, accurately and according to OASIS user’s manual definitions. Completes all documentation, patient health status reports as required and forwards appropriately.
• Supervises occupational therapy assistant according to state regulations and agency policy.
• Assists with the coordination of the total plan of care and maintains continuity of patient care by liaising with other appropriate staff assigned to the same patients. Participates in team/case conferences. Initiates team/care conferences for complex and/or multi

Requirements

• Must possess a current, unencumbered license to practice as a Physical Therapist in the State of New York.
• Successful completion of Physical Therapy program at a school of Physical Therapy approved by the American Physical Therapy Association or the Council on Medical Education and Hospitals of American Medical Association or the committee on Allied Health Education and Accreditation of the American Medical Association.
• Master’s degree or Doctorate degree in Physical Therapy.
• One year of Physical Therapy experience preferred.
