Job summary

Hirebridge, in partnership with The New Jewish Home Certified Home Health Agency (CHHA) in New York, seeks Fee for Service Physical Therapists to join our home health and rehabilitation team.

You will collaborate with physicians and the care team to implement plans of care, supervise PT aides, perform OASIS assessments, document progress, and coordinate the total plan of care to ensure seamless patient services in the community.

Responsibilities

• Collaborates with physicians, patients and their families, nurses, therapists, social workers and other members of the patient care team to develop and implement patient specific care and treatment plans.
• Utilizes varied solutions to educate and promote the health and independence of each patient.
• Assess, monitor, document and report progress of patients' health and condition using required documents, reports and established timelines. Provides supervision and evaluation of aides and physical therapy assistants.
• Observes signs and symptoms of patients, reports to the physician and/or other appropriate staff as often as needed or as change in the patients' condition requires.
• Completes OASIS assessments timely, accurately and according to OASIS user's manual definitions. Completes all documentation, patient health status reports as required and forwards appropriately.
• Supervises occupational therapy assistant according to state regulations and agency policy.
• Assists with the coordination of the total plan of care and maintains continuity of patient care by liaising with other appropriate staff assigned to the same patients. Participates in team/case conferences. Initiates team/care conferences for complex and/or multidisciplinary patients when required to ensure coordination of care.
