Job summary

BayMark Health Services is seeking a Compassionate Licensed Substance Use Counselor for outpatient care. The role involves individual and group counseling, crisis management, and collaboration with a medical team to support recovery from opioid use disorder.

You will assess progress, develop care plans, and coordinate services to ensure safe, trauma-informed treatment and successful discharge planning for patients.

Qualifications

• Must possess an Ohio CDCA, LCDC II, LCDC III, LPC, or LSW.
• Experience working with chemically dependent individuals preferred.
• Positive, non-judgmental and accepting attitude toward patients.
• Understanding of an EMR system (electronic medical records).
• Satisfactory drug screen and criminal background check.

Responsibilities

• Assist patients through medically assisted treatments and techniques for opioid addiction.
• Conduct individual and group sessions to support crisis management and coping strategies.
• Evaluate patients’ progress and collaborate with doctors, nurses and other counselors.
• Provide individual and group counseling services; coordinate all services during treatment.
• Establish and review initial treatment plans with each patient.
• Complete Bio-psychosocial assessments.
• Develop aftercare and discharge plans.
• Ensure delivery of counseling services in a trauma-informed manner.
• Perform other related duties as assigned by supervisor.

Position Overview

The Substance Use Counselor will be in charge with assisting patients through medically assisting treatments and offering techniques for handling opioid addiction. Counselor conducts individual sessions and group sessions to assist with crisis management and coping strategies. Counselor will evaluate patients’ progress during counseling and collaborate with doctors, nurses and other counselors to assist the patients’ achieving overall outcome.

Responsibilities

• Substance use counselor will be in charge of assisting patients through medically assisting treatments and offering techniques for handling opioid addiction.
• Counselor conducts individual sessions and group sessions to assist with crisis management and coping strategies.
• Counselor will evaluate patients’ progress during counseling and in collaborate with doctors, nurses and other counselors to assist the patients’ achieving overall outcome.
• Provide individual and group counseling services; direct service hours at regularly scheduled intervals; coordinate all services throughout treatment
• Facilitates, establishes and reviews with each patient the initial treatment plan
• Completes Bio-psychosocial assessment.
• Develop aftercare plans and discharge plans
• Ensure efficient and effective delivery of counseling services to all patientsin trauma informed manner
• Other related duties as determined by supervisor

Qualifications

• Previous experience working with chemically dependent individuals is preferred
