Job summary

Concho Health and Rehabilitation Center is seeking a dedicated Charge Nurse (LVN or RN) to join our care team. You will guide Nurse Aides, administer medications, and implement individualized care plans to support residents’ health and well-being.

We value reliability, strong clinical skills, and clear communication to ensure safe, coordinated care. This is a full-time on-site role with a $3,000 sign-on bonus available for eligible candidates from 10/19–11/18/2026.

Qualifications

• RN or LVN license in good standing.
• Strong clinical, assessment, and documentation skills.
• Excellent communication and ability to organize workflows.
• Responds efficiently to emergencies with attention to detail.

Responsibilities

• Direct care teams: guide Nurse Aides to ensure resident care.
• Administer medications as prescribed.
• Implement resident care plans per policies and orders.
• Perform treatments to maintain health and comfort.
• Transcribe physician orders accurately and promptly.
• Document resident care in charts and complete nursing assessments.
• Collaborate during shift reports for care continuity.
• Educate support staff on nursing policies and resident needs.
• Maintain safety standards and infection control.
• Address staffing needs by arranging replacements when needed.
