Signature HealthCARE of McCreary County Rehab & Wellness Center in Kentucky is seeking a dedicated Registered Nurse to deliver direct patient care, supervise nursing staff, and ensure high-quality resident outcomes. The role emphasizes compassionate care, team collaboration, and adherence to care plans.

You will coordinate admissions and discharges, document meticulously, and participate in quality assurance activities.

Qualifications

• Active RN license in state of practice.
• CPR/BLS certification preferred.
• Minimum one year of clinical nursing experience; long-term care experience preferred.
• Experience in a supervisory or charge nurse role preferred.
• Strong clinical observation, medication administration, and documentation skills.
• Familiarity with electronic health records (EHR).
• Proficiency with Microsoft Office applications.

Responsibilities

• Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
• Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety at all times.
• Supervise and direct nursing assistants and medication aides during the assigned shift in accordance with facility policies and regulations.
• Monitor and observe residents’ physical and emotional status; notify the attending physician and responsible party of any change in condition.
• Develop, implement, and review individualized resident care plans in collaboration with the interdisciplinary team.
• Complete accurate and timely clinical documentation per facility policy and procedure.
• Coordinate resident admissions, transfers, and discharges according to established procedures.
• Participate in quality assurance activities, staff education, and regulatory surveys as needed.
• Perform other duties as assigned by the Director of Nursing or designee.
