Job summary

Harbor Lakes Nursing & Rehabilitation in Granbury, TX seeks a licensed LVN to join our Charge Nurse team. The role focuses on compassionate nursing care, medication administration, and monitoring residents under RN/physician supervision.

You will collaborate with interdisciplinary staff to implement individualized care plans, document changes, and support daily activities while upholding safety and infection control standards. This is a full-time, hourly position with shift differentials.

Qualifications

• Current and valid LVN license in Texas.
• Completion of an accredited LVN program.
• BLS certification.

Responsibilities

• Provide direct nursing care and administer medications as prescribed.
• Monitor vital signs and document changes; report significant findings.
• Assist residents with activities of daily living and mobility.
• Collaborate with RNs and physicians to update care plans.
• Maintain accurate patient records in line with policies.
