Job summary

Holland Lake Rehabilitation & Wellness Center in Weatherford, TX is seeking a full-time licensed RN/LVN to lead nursing operations on a day-to-day basis. Responsibilities include direct resident care, medication administration, and ensuring adherence to physician orders and care plans.

The role requires a Texas license (RN or LVN), CPR certification, and at least one year of experience in hospital or long-term care settings.

Qualifications

• Current Texas RN or LVN license and a nursing degree.
• CPR certification required.
• At least 1 year experience in hospital or long-term care.
• Strong communication, leadership and teamwork required.

Responsibilities

• Provide hand on care to the resident based on the resident Plan of Care.
• Administer medications and provide treatment to the resident based on the physician orders.
• Must be able to identify changes in the resident condition and evaluate the resident care needs.
• Strong focus on assessment and documentation skills.
• Must be able to evaluate and participate in the development of the Resident Plan of Care.
• Monitor the resident overall condition and provide care as required while complying with facility policies.

Requirements

• An active Texas RN/LVN License and nursing degree from an accredited school of nursing, college or university along with a CPR certification
• A minimum of one year experience in a hospital, long term care facility or other related health care facility.
