Holland Lake Rehabilitation & Wellness Center in Weatherford, TX is seeking a motivated nurse leader for a full-time/part-time management role. You will oversee day-to-day nursing operations, lead a large care team, and ensure high-quality resident care in alignment with the Plan of Care.

Candidates must hold a Texas RN or LVN license with a nursing degree and CPR certification, plus at least one year of relevant facility experience. Excellent communication and analytical skills are essential.

• Active Texas RN/LVN license with nursing degree and CPR certification.
• Minimum one year of experience in hospital or long-term care facility.

Responsibilities:

• Provide hand on care to the resident based on the resident Plan of Care.
• Administer medications and provide treatment to the resident based on the physician orders.
• Must be able to identify changes in the resident condition and evaluate the resident care needs.
• Strong focus assessment skills as well as documentation skills.
• Must be able to evaluate and participate in the development of the Resident Plan of Care.
• Monitor the resident overall condition and provide care as required while maintaining compliance with the facility policy and procedures.

Qualifications

• Current RN/LVN License in Texas
• Excellent interpersonal, written and verbal communication skills
• Strong leadership skills and ability to work collaboratively with a large team
• Able to work efficiently under time constraints
• Strong analytical and mathematical skills
• Ability to handle multiple assignments simultaneously

Requirements:

• An active Texas RN/LVN License and nursing degree from an accredited school of nursing, college or university along with a CPR certification
• A minimum of one year experience in a hospital, long term care facility or other related health care facility.
