Job summary

La Vida Serena Nursing and Rehabilitation is seeking a Charge Nurse (RN or LVN) to lead care teams and ensure high-quality resident care on-site. You will administer medications, implement care plans, and document nursing assessments while guiding staff through policies and procedures.

Ideal candidates hold an RN or LVN license in good standing, with strong assessment, documentation, and communication skills, and the ability to respond quickly in emergencies.

Responsibilities

Your Impact as a Charge Nurse - LVN or RN

In this role, you will:

• Direct Care Teams: Provide guidance to Nurse Aides to ensure appropriate care for residents.
• Administer Medications: Properly and safely administer resident medications as prescribed.
• Implement Care Plans: Execute resident care plans in compliance with company policies, federal guidelines, and physician orders.
• Perform Treatments: Deliver treatments as required to ensure resident health and comfort.
• Transcribe Orders: Accurately and promptly transcribe physician orders.
• Document Resident Care: Maintain timely and accurate documentation in resident charts and complete required nursing assessments.
• Collaborate During Shift Reports: Participate in shift reports to ensure seamless communication and care continuity.
• Educate Support Staff: Implement nursing policies and procedures, educating staff on the care needs of residents.
• Maintain Safety Standards: Promote a safe working environment by complying with infection control and body mechanic procedures.
• Address Staffing Needs: Respond to staffing shortages by arranging for replacement staff as necessary.
