Job summary

Riverside Oaks in Victoria, TX is seeking an LVN/LPN (Licensed Vocational/Practical Nurse) to join a dedicated nursing team in a Skilled Nursing setting. This PRN position offers the opportunity to participate in comprehensive patient care within the scope of licensure and to contribute to high-quality clinical outcomes.

The role focuses on accurate patient assessment, timely documentation, and collaboration with CNAs and other staff to implement care plans and ensure well-being.

Qualifications

• Graduation from an accredited Nursing School.
• Valid LVN/LPN license required.

Responsibilities

• As an LVN/LPN, you will participate in and oversee patient care within the scope of the LVN/LPN licensure.
• Observe, assess, and report patient conditions/changes and document in accordance with Cantex’s Clinical Policies & Procedures and state/federal guidelines & regulations.
• EMR documentation relevant to patient assessment, evaluation, change of condition, in accordance to nursing practices.
• Ensure implementation of care plans as directed and administer medications and treatments as prescribed.
• Participate in assigning duties and supervise the delivery of appropriate clinical care by the CNA staff to ensure highest practical well-being is maintained by patient.
