Job summary

Regency Integrated Health Services seeks an LVN Charge Nurse at Windsor Mission Oaks in San Antonio, TX to help deliver quality resident care. You will observe, document, administer medications, and coordinate with physicians; supervise aides; complete nursing assessments and discharge residents according to policy.

Texas Licensed Vocational Nurse in good standing is required; SNF experience is preferred. This is a full-time, on-site opportunity with Riverside facility policies in place.

Qualifications

• Has completed an LVN program at an accredited vocational school or college.
• Active Licensed Vocational Nurse in Texas in good standing.
• Experience working in a skilled nursing facility is preferred.

Responsibilities

• Observe, assess, document, and report resident conditions during rounds.
• Assure implementation of care plans as directed.
• Administers medications as prescribed.
• Conduct rounds with physician and record visits in clinical records.
• Perform narcotic reconciliation with incoming and outgoing shifts.
• Identify significant changes in residents' condition and act per policy.
• Oversee and monitor aides.
• Initiate and complete Incident and Accident Reports per policy.
• Discharge residents per policy.
• Complete Nursing Assessments prior to physician calls and communicate status updates.
