Job summary

Kindred Hospital Louisville is seeking an experienced LPN/LVN Charge Nurse to oversee a nursing unit under the direct supervision of a Registered Nurse. You will lead a designated team of RNs, LPNs, and aides ensuring compliant, high‑quality patient care during your shift.

Responsibilities include coordinating admissions, documenting care in the EHR, managing staffing, and mentoring staff while upholding PHI privacy and hospital policies.

Responsibilities

• Assesses, plans, coordinates and evaluates patients’ care along with the interdisciplinary team.
• Documents the patient’s condition and nursing needs in the EHR promptly and accurately.
• Reports observations to clinical leadership and Physicians as necessary.
• Receives and records physicians’ orders.
• Manages staffing and deployment; provides accurate patient status reports.
• Develops work assignments for RNs, LPN/LVNs, and Nurse Aides; supervises staff.
• Participates in counseling, disciplinary actions and performance appraisals as allowed.
• Ensures adherence to policies,PHI confidentiality and performance improvement initiatives.
• Initiates SBAR and collaborates with staff for assessments and recommendations.
• Makes rounds with physicians and other team members as needed.
• Coordinates admissions, transfers and discharges; completes assessments.
• Assists with pain management and rehabilitation as per plan of care.
• Interacts professionally with patients and families and respects cultural needs.
