Job summary

Vitality Living is seeking an experienced LPN/LVN to join our team and promote successful aging. You will administer medications, monitor vital signs, and coordinate with physicians to ensure accurate documentation and patient safety.

Responsibilities include supervising care staff, updating service plans, and participating in onboarding and ongoing training. A valid LPN/LVN license and CPR certification are required, along with strong communication skills in English.

Qualifications

• Must be at least 21 years old.
• Demonstrate ability to communicate effectively in English, both verbally and in writing.
• Meet state or provincial health related requirements.
• Maintain current professional license if LPN/LVN.
• Maintain current CPR certification if LPN/LVN.
• Maintain any other certifications as required by state or provincial regulations.

Responsibilities

• Administer or assist residents to self-administer medications and treatments.
• Measure vital signs and weight.
• Communicate with physicians via telephone, fax, and Physician Communication forms.
• Ensure physician orders, laboratory reports, and other data are scanned and filed in resident charts.
• Read Shift Change Notes and communicate pertinent information to Wellness Director.
• Attend onboarding, annual and in-service training and team meetings.
