Emerald Care Center Claremore is seeking a Licensed Practical Nurse (LPN) to join our team in Claremore, OK. You will assess resident needs, provide direct nursing care, and collaborate with the healthcare team to achieve the best outcomes for our residents.

Responsibilities include administering medications, wound care, monitoring vital signs, and contributing to individualized care plans in a supportive, home-like environment.

Qualifications

• Valid LPN/LVN license in Oklahoma.
• Graduation from an accredited LPN/LVN program.
• Experience in a skilled nursing facility or similar setting preferred.
• Current CPR/BLS certification required.
• Strong communication and teamwork skills.
• Ability to lift and move patients and stand for extended periods.

Responsibilities

• Provide direct nursing care to residents, including administering medications, wound care, and treatments as prescribed by the physician.
• Monitor residents’ vital signs, document progress, and report changes to the Charge Nurse or Director of Nursing.
• Assist in development and implementation of individualized care plans.
• Maintain clear and accurate documentation of patient care, and communicate with residents, families, and the healthcare team.
• Adhere to facility policies, procedures, and regulatory requirements, including infection control and safety protocols.
• Provide emotional support and education to residents and families about care plans and health conditions.
• Collaborate with RNs, CNAs, physicians, and therapists to deliver coordinated care.

Job Summary:

As a Licensed Practical Nurse (LPN) at Emerald Care Center Claremore, you will play a crucial role in delivering high-quality, compassionate care to our residents. • Communication: Maintain clear and accurate documentation of patient care, communicate effectively with residents, families, and the healthcare team.
• Compliance: Adhere to all facility policies, procedures, and regulatory requirements, including infection control and safety protocols.
• Support: Provide emotional support and education to residents and their families, helping them understand care plans and health conditions.
• Team Collaboration: Work collaboratively with other healthcare professionals, including RNs, CNAs, physicians, and therapists, to deliver coordinated and comprehensive care.

Qualifications:

• Licensure: Valid LPN/LVN license in Oklahoma
• Education: Graduation from an accredited LPN/LVN program
• Experience: Previous experience in a skilled nursing facility or similar healthcare setting preferred
• Certifications: Current CPR/BLS certification required. Additional certifications may be required based on state regulations
• Communications: Excellent verbal and written communication skills. Ability to work collaboratively with a diverse team of healthcare professionals and build positive relationships with residents and their families
• Physical Requirements: Ability to lift and move patients, perform physical assessments, and stand for extended periods
