Job summary

Qualifications

• Active Licensed Vocational Nurse in Texas in good standing.
• Experience in a skilled nursing facility preferred.

Responsibilities

• Observe, assess, document, and report resident condition/changes during rounds.
• Ensure implementation of care plans as directed.
• Administer medications as prescribed.
• Coordinate with physicians during rounds and document in clinical records.
• Perform narcotic reconciliation with incoming/outgoing shifts.
• Discharge residents per facility policy.
• Communicate with residents' designated contacts about status or changes in condition.
