Job summary

Gulf Pointe Plaza Skilled Nursing & Rehab in Rockport, TX, is seeking a Licensed Vocational Nurse (LVN) for PRN shifts. You will supervise CNAs, develop and implement individual care plans, administer nursing care, and coordinate with physicians to ensure high-quality resident outcomes.

Qualified candidates hold an LVN license in good standing, have at least 1 year of nursing experience (long-term care preferred), and demonstrate strong leadership, organization, and documentation skills.

Qualifications

• LVN license, in good standing.
• 1 year of nursing experience; long-term care experience preferred.
• Ability to organize, prioritize and maintain confidentiality.
• Strong supervisory and training skills.
• Case management and client relations experience.

Responsibilities

• Supervise the nursing unit and coordinate personnel, supplies, equipment and facilities.
• Review staffing patterns and census; reassign staff as needed.
• Assess new admissions and create care plans for residents.
• Administer medications, IV therapy, gastronomy tube management and some wound care.
• Monitor residents and communicate changes to physicians.
• Ensure accurate documentation of resident care and maintain medical records.
• Troubleshoot and resolve complaints as necessary.
