Job summary

TN Quality Care Hospice is seeking a PRN Chaplain to join our Columbia, TN team. You will provide direct spiritual care and bereavement counseling to patients and families while supporting the interdisciplinary care team.

As a PRN role, you will participate in continuous care, on-call duties, and coordinate bereavement services post-patient death as needed.

Qualifications

• Graduate of an accredited seminary or school of theology; certificate of ordination or commission preferred.
• Valid drivers license and proof of insurance.

Responsibilities

• Performs spiritual bereavement assessments for patients and families and reflects needs in the plan of care.
• Provides direct spiritual care and bereavement counseling to patients and families.
• Provides consultation and support to the interdisciplinary team on spiritual care and bereavement.
• Participates in Continuous Care and on-call as needed.
• Assists in supervision of spiritual care volunteers when assigned.

Qualifications

Graduate of an accredited seminary or school of theology andor a certificate of ordination or commission from a religious faith group; completion of Clinical Pastoral Education CPE preferredPrevious hospice experience working with death and dying strongly preferredComfortable in an interfaith setting and able to accept different lifestyles cultures beliefs and valuesValid drivers license and proof of insurance is required

What Youll Do

• Performs spiritual bereavement assessments of patients and familiescaregivers in the hospice program in a timely manner and ensure that identified needs are addressed in the patients plan of care
• Provides direct spiritual care and bereavement counseling to patients and familiescaregivers
• Demonstrates exceptional service to patients families referral sources co workers and other customers
• Assists team members as needed to ensure that this level of service is consistently provided
• Provides accurate and timely documentation of patient services to reflect the plan of care
• Attends group meetings to facilitate coordination of care and discussion of interdisciplinary group involvement
• Provides consultation education and support to the interdisciplinary group on spiritual care and bereavement counseling
• Participates in Continuous Care and on call if necessary
• Assists in supervision of spiritual care volunteers when assigned to patients familiescaregivers as needed
• Assumes responsibility in directing the development and implementation of the bereavement plan of care and the frequency of service delivery; make available bereavement services for up to one year following the death of the patient
