Job summary

Addus HomeCare in Columbia, TN is seeking a PRN Chaplain to join our hospice team. Shifts are as needed, typically weekdays, with location in Columbia, TN and surrounding counties.

You will provide spiritual care, bereavement support, and coordinate with an interdisciplinary team to ensure compassionate, patient-centered care. A seminary graduate with interfaith sensitivity and valid driver’s license is required.

Qualifications

• Graduate of accredited seminary or school of theology; ordination/commission from a religious faith group; CPE preferred.
• Hospice experience with death and dying strongly preferred.
• Comfortable in an interfaith setting and able to accept diverse beliefs, cultures, and values.
• Valid driver’s license and proof of insurance required.

Responsibilities

• Performs spiritual bereavement assessments of patients and families in the hospice program in a timely manner and ensures identified needs are addressed in the plan of care.
• Provides direct spiritual care and bereavement counseling to patients and families/caregivers.
• Demonstrates exceptional service to patients, families, referral sources, co-workers, and other customers; assists team members as needed.
• Provides accurate and timely documentation of patient services to reflect the plan of care.
• Attends group meetings to facilitate coordination of care and interdisciplinary discussions.
• Provides consultation, education, and support on spiritual care and bereavement counseling to the interdisciplinary group.
• Participates in Continuous Care and on-call duties, if necessary.
• Assists in supervising spiritual care volunteers when assigned to patient families/caregivers.
• Directs the development and implementation of the bereavement plan of care and offers bereavement services for up to one year after patient death.
