Job summary

Alive Hospice in Lebanon, TN seeks a Chaplain to provide spiritual care to patients and families in a compassionate, interdisciplinary team setting. This PRN role focuses on comfort, bereavement support, and faith-based rites in alignment with patient wishes.

The Chaplain will conduct spiritual assessments, collaborate with the care team, lead services when requested, and educate staff on diverse beliefs while ensuring timely documentation and respectful, dignified end-of-life care.

Qualifications

• Master of Divinity or equivalent that provides spiritual care and counseling.
• Minimum of one unit of Clinical Pastoral Education (CPE) required.
• Current driver’s license and automobile insurance as required by Tennessee state law.

Responsibilities

• Completes initial spiritual assessment of patient and family needs per agency standards.
• Develops care plan with interdisciplinary team, patient, and family; include faith community if requested.
• Visits patients and families, providing spiritual counsel and care.
• Engages in prayer and sacramental ministry as requested and appropriate.
• Plans and conducts funerals or memorial services as requested.
• Assists patients/families in crisis.
• Communicates with patient’s clergy or faith community as appropriate.
• Provides bereavement support and counsel for patients, families and staff.
• Educates staff on beliefs and rituals of different faiths.
• Participates in interdisciplinary care planning conference and team meetings.
• Provides community education related to local clergy and church groups.
• Completes clinical documentation in a timely manner per Alive Hospice procedures and regulations.
• Facilitates services in patient room, home, or quiet room as requested.
• Attends department and discipline meetings and in-services as scheduled.
• Participates in memorial services and agency events as requested or assigned.
• Participates in on-call schedule rotation.

Summary

The Chaplain provides spiritual counsel and support to the hospice patients and their families.

Essential Duties And Responsibilities

• Completes an initial spiritual assessment of patient\'s and family’s needs according to agency standards and regulatory requirements.


• Develops and implements a plan of care with members of the interdisciplinary team, the patient and family; and includes the patient\'s faith community if requested.


• Visits patients and families, providing spiritual counsel and care in a manner accessible to and agreeable to the individual patient and family.


• Engages in prayer and sacramental ministry to patients and families, as requested, and as appropriate.


• Plans and conducts funerals or memorial services, as requested by patients and/or families.


• Assists with patients/families in crisis.


• Communicates with patient\'s clergy or faith community as appropriate.


• Provides bereavement support and counsel for patients, families and staff


• Educates staff on beliefs and rituals of different faiths.


• Participates in interdisciplinary care planning conference and team meetings.

Requirements

EDUCATION and/or EXPERIENCE

Master of Divinity Degree or equivalent graduate degree that provides advanced education to provide spiritual care and counseling. Minimum of one (1) unit of Clinical Pastoral Education (CPE) required.

Current driver\'s license and automobile insurance as required by Tennessee State Law.
