Job summary

Bristol Hospice in New Albany, IN is seeking a Chaplain/Spiritual Care Provider to deliver compassionate spiritual assessments and develop individualized care plans. This PRN role involves travel to assigned hospice locations and close collaboration with the Interdisciplinary Team to support patients and families.

The position requires documentation of services in the EMR and a theology degree. Prior hospice experience is preferred and flexibility with hours is expected.

Qualifications

  • Graduate of an accredited seminary or theology program with relevant degree.
  • Experience in hospice or death/dying care preferred.
  • Ability to travel between hospice locations as required.

Responsibilities

  • Assure spiritual assessment of patients and families and provide timely services.
  • Develop individualized plans of care in collaboration with the Interdisciplinary Team.
  • Deliver direct spiritual care to patients and families/caregivers.
  • Document services and communicate with community chaplains and spiritual counselors.
  • Participate in care conferences and quality improvement activities.
