Job summary

Bristol Hospice is hiring an experienced Hospice Chaplain to join our team in Charleston, SC. The role focuses on spiritual assessments, planning individualized care, and providing direct spiritual support to patients and families.

Travel to hospice locations is expected, with day shifts and a salary range of $65,000 to $70,000 annually. Join a mission-driven organization committed to compassionate care and family-centered hospice services across communities.

Qualifications

• Graduate theology degree preferred.
• Experience working with death and dying, individuals/family/caregiver.
• Hospice experience preferred.
• Willingness to travel to hospice locations as assigned and flexible with working hours.
• Proficient in MS Word, Excel, PowerPoint and EMR systems.

Responsibilities

• Assure spiritual assessment of patients and families in the hospice program and ensure services are rendered timely.
• Facilitate development of individualized plan of care by participating in comprehensive assessment to meet spiritual needs.
• Provide direct spiritual care to patients and families/caregivers.
• Serve as liaison and support to community chaplains and spiritual counselors.
• Maintain records of spiritual care services utilization and related activities for QA and program development.
• Document direct services and ongoing communication with community chaplains and spiritual counselors.
• Attend patient care conferences and interdisciplinary group meetings as a member of the team.
• Recruit community chaplains and spiritual care volunteers by developing community contacts.

Education

Graduate of an accredited seminary or school of theology, BD, M.Div. or equivalent theological degree preferred

QualificationsOn an Average Day You Will:

• Assure spiritual assessment of patients and families/caregivers in the hospice program and appropriate services are rendered in a timely manner


• Facilitate the development of the individualized plan of care by participating in the comprehensive assessment to meet identified spiritual needs


• Provide direct spiritual care to patients and families/caregivers


• Serve as liaison and support to community chaplains and spiritual counselors


• Maintain records of spiritual care services utilization and related activities for quality assurance, program development, and policies and procedures review and revision


• Document direct services and ongoing communication with community chaplains and spiritual counselors


• Attend patient care conference and interdisciplinary group meetings as a member of the interdisciplinary group


• Provide consultation, education and support to interdisciplinary group on spiritual care


• Recruit community chaplains, spiritual counselors and spiritual care volunteers adequate to meet patient and family/caregiver needs by developing community contacts and offering education through congregations about hospice care


• Provide for funeral or memorial services for patients as requested


• Plan periodic memorial services to meet the needs of personnel, volunteers and community clergy/spiritual counselors working with the hospice team


• Assist in supervision of spiritual care volunteers when assigned to patients/families/caregivers


• Actively participate in quality assessment performance improvement teams and activities


• Accept and perform other related duties and responsibilities, as required


• Other duties as assigned

Requirements:
