Job summary

Lightways Hospice and Serious Illness Care seeks a Pediatric Chaplain to provide spiritual care for patients and families, coordinating with other spiritual counselors as needed. The role includes documentation, participation in care conferences, and collaborative planning within the hospice team.

The position requires a seminary or theology degree, Clinical Pastoral Education, and health-care experience with palliative or hospice settings.

Qualifications

  • Graduate of an accredited seminary or school of theology (BD, MDiv or equivalent theological degree).
  • Clinical pastoral education certification required.
  • Minimum of one year’s experience in health care; palliative or hospice experience preferred.
  • Must be a licensed driver with an automobile that is insured and in good working order.

Responsibilities

  • Assesses spiritual needs of patients and families in the hospice program and ensures timely services.
  • Contributes to the individualized plan of care through comprehensive spiritual assessments.
  • Serves as liaison and support to community chaplains and spiritual counselors.
  • Maintains records of spiritual care services for QA and program development.
  • Documents direct services and ongoing communication with community chaplains and counselors.
  • Attends patient care conferences and interdisciplinary team meetings as a member of the team.
  • Monday through Friday schedule with on-call responsibilities.

Job description

Pediatric Chaplain role focused on providing spiritual care to patients and families/caregivers of Lightways Hospice and Serious Illness Care, coordinating with other spiritual counselors as needed.

Key Responsibilities
  • Assesses spiritual needs of patients and families/caregivers in the hospice program and ensures appropriate services are rendered in a timely manner.
  • Contributes to the development of the individualized plan of care by participating in the comprehensive assessment to meet identified spiritual needs.
  • Serves as liaison and support to community chaplains and spiritual counselors.
  • Maintains records of spiritual care services utilization and related activities for quality assurance, program development, and policies and procedures review and revision.
  • Documents direct services and ongoing communication with community chaplains and spiritual counselors.
  • Attends patient care conferences and interdisciplinary team meetings as a member of the interdisciplinary group.
  • Monday through Friday schedule with on-call responsibilities.
Education/Experience
