Job summary

VIA Health Partners seeks a compassionate Chaplain to serve as the spiritual care resource for the interdisciplinary team, coordinating care plans and supporting patients and families at end of life. You will provide religious rites, prayer, and spiritual guidance aligned with individual beliefs.

In this role, you may also offer grief care and collaborate with other disciplines to ensure holistic, patient-centered care."

Summary

The chaplain serves as the spiritual care professional resource for the interdisciplinary team. Responsible for providing spiritual care to individuals and their families through assessment, goals, interventions, and specific care at time of death. Coordinates plan of care by working to anticipate and meet patient needs and address specific plan of care goals. Works closely with related disciplines to ensure professional service delivery. May provide grief care if in a dual chaplain/grief care role.
