Job summary

Affinity Hospice in Lawrenceville, GA is seeking a Chaplain to join our interdisciplinary team, providing compassionate spiritual care to patients and families at end of life.

The role requires a seminary degree, CPE units, a valid driver’s license, and the ability to collaborate with community clergy while honoring diverse faith traditions and hospice philosophy.

Qualifications

  • Graduate of accredited seminary or school of theology (Bachelor of Divinity, MasterofDivinityorequivalent theological degree).
  • Chaplain Pastoral Education (CPE) Units (Varies by State)
  • Licensed driver with automobile thatisinsuredinaccordancewithorganizationrequirements and is in good working order.
  • Must understand and follow company policies andprocedures
  • Must be able to network with community clergy and congregations and present a professional image
  • Experience working with death and dying,individuals/family/caregiver
  • Ability to work as member of interdisciplinary group
  • Comfort in an interfaith setting
  • Ability to accept different lifestyles, cultures, beliefs, and values
  • Knowledge of and commitment to hospice philosophy of care
  • CPR Certification

Responsibilities

  • Assures spiritual assessment and spiritual care of patients and families/caregivers in the hospice program andappropriateservicesarerenderedina timely manner
  • Facilitates the development of theindividualizedtheplanof carebyparticipatinginthe comprehensive assessment tomeetidentifiedspiritualneedsServesas liaison andsupport tocommunity chaplains and spiritualcounselors
  • Maintains records of spiritual careservicesutilizationandrelated activities for quality assurance, program development, and policies and procedures review and revision
  • Documents direct services and ongoing communication with community chaplains and spiritualcounselors
  • Attends patient care conference and interdisciplinary group meetings as a member of the interdisciplinary group
  • Provides consultation, education, and support to the interdisciplinary group on spiritual care
  • Recruitscommunitychaplains, spiritual counselors, and spiritual care volunteers adequate to meet patient and family/caregiver needs by developing community contacts and offering education through congregations about hospice care
  • Provides for funeral or memorial services for patients asrequested
  • Plans periodic memorial services to meet the needs of associate, volunteers, and community clergy/spiritual counselors working with the hospice team
  • Assistsin supervision of spiritual care volunteers when assigned to patients/families/caregivers
  • Activelyparticipatesinquality assessment performance improvement teams andactivities
  • Accepts and performs other related duties and responsibilitiesasrequired

Position Summary:

The Chaplain serves as an integral member of the interdisciplinary team, concentrating on the spiritual care of patients and their families/caregivers. The Chaplain is committed to achieving excellence in pastoral care,necessitatinga genuine presence, empathy, andproficiencyin navigating the spiritual complexities associated with illness, loss, and death.

Hours / Schedule:

Full Time; (40) hours / week

Minimum Qualifications:
