Job summary

Alternate Solutions Health Network is seeking a Hospice Chaplain to provide spiritual care and emotional support to patients and families within our hospice program.

The role collaborates with the Interdisciplinary Group, documents care, coordinates with chaplains, and facilitates memorial services.

Qualified candidates will be ordained or licensed, with at least one year in hospice or healthcare chaplaincy, and a degree history; a driver license is required.

Qualifications

  • Ordained, licensed, or endorsed by a recognized religious body
  • Minimum of one year of experience in hospice, palliative care, or healthcare chaplaincy
  • Completion of at least one unit of Clinical Pastoral Education (CPE) preferred
  • Strong interpersonal skills, empathy, and the ability to provide nonjudgmental support
  • Ability to work with individuals of all faiths, belief systems, and cultural backgrounds
  • Valid driver’s license and reliable transportation required

Responsibilities

  • Conduct spiritual assessments and provide individualized spiritual support to hospice patients and their families
  • Participate in the development and implementation of the interdisciplinary plan of care based on identified spiritual needs
  • Provide direct spiritual counseling and support to patients, families, and caregivers
  • Coordinate with community chaplains, clergy, and spiritual counselors to extend care when appropriate
  • Maintain records of spiritual care services for quality assurance and documentation purposes
  • Document all services, coordination activities, and patient communications in the electronic health record system
  • Attend and participate in patient care conferences and interdisciplinary group (IDG) meetings
  • Educate and consult with the care team regarding spiritual concerns and religious diversity
  • Recruit and develop relationships with local faith-based organizations and clergy to support patient and family needs
  • Provide or coordinate funeral, memorial, or celebration of life services upon request
  • Plan periodic memorial services to support families, team members, volunteers, and community clergy
  • Support and guide spiritual care volunteers, providing supervision as needed
  • Maintain confidentiality and demonstrate cultural humility and respect in all interactions
  • Perform other duties as assigned

SUMMARY

The Hospice Chaplain provides spiritual care and emotional support to hospice patients and their families/caregivers. This role is a vital part of the interdisciplinary group, working closely with other care team members to ensure each patient’s spiritual needs are met with compassion, respect, and sensitivity. The chaplain also serves as a liaison to community faith leaders and may facilitate memorial and bereavement services.

KEY RESPONSIBILITIES
