Job summary

Bristol Hospice in Tucson, AZ is seeking an experienced Hospice Chaplain to join our team. The role focuses on spiritual assessments, creating care plans, and working closely with the Interdisciplinary Team to meet patient and family needs.

The ideal candidate holds a seminary degree, has hospice experience, and is proficient with MS Word, Excel, PowerPoint, and EMR systems. This full-time, day-shift position offers competitive hourly compensation and a mission-driven work culture.

Qualifications

  • Graduate of an accredited seminary or school of theology (BD, M.Div. or equivalent preferred).
  • Experience with death and dying patients and families.
  • Hospice experience preferred.
  • Proficient with Word, Excel, PowerPoint and EMR systems.

Responsibilities

  • Assure spiritual assessment of patients and families/caregivers in the hospice program and ensure appropriate services are rendered in a timely manner.
  • Facilitate the development of the individualized plan of care by participating in the comprehensive assessment to meet identified spiritual needs.
  • Provide direct spiritual care to patients and families/caregivers.
  • Serve as liaison and support to community chaplains and spiritual counselors.
  • Maintain records of spiritual care services utilization and related activities for quality assurance, program development, and policy review.
  • Document direct services and ongoing communication with community chaplains and spiritual counselors.
  • Attend patient care conference and interdisciplinary group meetings as a member of the interdisciplinary group.
  • Provide consultation, education and support to the interdisciplinary group on spiritual care.
  • Recruit community chaplains, spiritual counselors and spiritual care volunteers to meet patient and family/caregiver needs by developing community contacts and offering education through congregations about hospice care.
  • Arrange funeral or memorial services for patients as requested.
  • Plan periodic memorial services to meet the needs of personnel, volunteers and community clergy/spiritual counselors working with the hospice team.
  • Assist in supervision of spiritual care volunteers assigned to patients/families/caregivers.
  • Actively participate in quality assessment performance improvement teams and activities.
  • Accept and perform other related duties and responsibilities as required.

Requirements
