Job summary

Image Strategic Partners in Pueblo, CO is seeking a Chaplain to provide spiritual care to hospice patients and families, integrating belief systems into end-of-life plans.

You will assess spiritual needs, develop care plans with the interdisciplinary team, offer bereavement support, educate staff and community groups, and participate in ongoing professional development.

Qualifications

• Two years of experience in pastoral field.
• Bachelor's degree.
• Licensed/ordained clergy; driver’s license and proof of current auto liability insurance.
• Community based bereavement and pastoral support experience a plus.
• Previous hospice experience a plus but not necessary

Responsibilities

• Provide spiritual support to patients, caregivers, and the Hospice Interdisciplinary team.
• Develop a comprehensive care plan with direct spiritual support and end-of-life counsel per patient beliefs.
• Contribute to comfort and emotional well-being during end-of-life journey; support diverse spiritual backgrounds.
• Collaborate with a multidisciplinary team and pursue ongoing professional development.
• Educate staff, clergy, and community groups on the importance of spiritual support at end-of-life.

Job Duties

• You will play an important role in providing spiritual support to patients, caregivers, and the Hospice Interdisciplinary team.
• By listening to your patient’s needs and completing a spiritual assessment, you will develop a comprehensive care plan that includes direct spiritual support and end-of-life counsel in accordance with their beliefs.
• Your work will provide increased comfort, reduced anxiety, and improved emotional and spiritual well-being to patients during their end-of-life journey. You will find your job rewarding and fulfilling as you support patients on your spiritual journey and gain valuable experiences in caring for patients from diverse spiritual and cultural backgrounds.
• Working alongside a talented multi-disciplinary team, you can expand your skillset and knowledge through ongoing professional development opportunities.
