Job summary

Grace Healthcare Services seeks a Chaplain to support patients, families, and caregivers through the emotional and spiritual journey of hospice care. As a key member of the Interdisciplinary Team (IDT), the Chaplain provides spiritual guidance, counseling, and emotional support tailored to individual beliefs and values.

The role emphasizes conducting spiritual assessments, offering pastoral care, and collaborating with IDT to ensure a holistic approach to patient care and memorial service

Qualifications

  • Master's degree in Divinity, Theology or related field (Preferred).
  • Experience in Healthcare or Hospice (Preferred) - Two or more years.
  • Clinical Pastoral Education (CPE) Training (Preferred) - At least one unit.
  • Strong Communication and Interpersonal Skills.
  • Compassionate and Authentic Presence.

Responsibilities

  • Provide Spiritual and Emotional Support—conduct spiritual assessments and pastoral care aligned with patient and family beliefs.
  • Be a Comforting Presence—visit patients regularly, offer compassion and prayer as desired.
  • Facilitate Connections—work with community clergy to ensure faith-based support.
  • Participate in Care Planning—collaborate with IDT for holistic patient care.
  • Lead Memorial Services—conduct or assist with memorial and funeral services.

About the Role

We are seeking a Chaplain to support patients, families, and caregivers through the emotional and spiritual journey of hospice care. As a key member of the Interdisciplinary Team (IDT), the Chaplain provides spiritual guidance, counseling, and emotional support tailored to the beliefs and values of each individual.

What You'll Do
  • Provide Spiritual and Emotional Support - Conduct spiritual assessments and offer pastoral care that aligns with the unique needs and beliefs of each patient and their family.
  • Be a Comforting Presence - Visit patients regularly, offering compassionate care and prayer as desired. Celebrate birthdays and special moments with patients and their families.
  • Facilitate Connections - Work with community clergy and religious organizations to ensure patients have access to the faith-based support they need.
  • Participate in Care Planning - Collaborate with the IDT to ensure a holistic approach to patient care.
  • Lead Memorial Services - Offer support by conducting or assisting with memorial and funeral services as requested.
What We're Looking For
  • Master's degree in Divinity, Theology, or a related field (Preferred)
  • Experience in Healthcare or Hospice (Preferred) - Two or more years in a similar role.
  • Clinical Pastoral Education (CPE) Training (Preferred) - At least one unit completed.
  • Strong Communication and Interpersonal Skills - Ability to connect with individuals from all backgrounds and faith traditions.
  • Compassionate and Authentic Presence - A heart for service and a deep understanding of grief, loss, and end-of-life care.
