Job summary

Gentiva is seeking a compassionate Hospice Chaplain to provide spiritual and emotional care to patients and families in alignment with diverse beliefs. You will join an interdisciplinary team and contribute to comfort and meaning at life's end.

Responsibilities include conducting visits, developing individualized care plans, facilitating rituals, and supporting bereavement. A theology-related degree and ACPE CPE are preferred.

Qualifications

  • Bachelor’s degree in theology, religion, human services, counseling, psychology, or sociology (or state requirements).
  • Minimum of one unit ACPE-accredited CPE (Clinical Pastoral Education).
  • Degrees from accredited institutions preferred; AVPO approval for non‑accredited degrees.

  • Provide spiritual and emotional support to patients, families, and caregivers in line with beliefs.
  • Offer non-denominational, inclusive care that respects all faiths and cultural backgrounds.
  • Be an integral member of the interdisciplinary team, attending care conferences with nurses, social workers, and medical staff.
  • Assess spiritual needs and develop individualized spiritual care plans.
  • Conduct visits, prayers, blessings, and rituals per patient/family requests.
  • Assist families during grief and bereavement, providing counseling and support.
  • Build relationships with local faith leaders and community resources.
  • Maintain timely documentation of spiritual assessments, interventions, and outcomes.

What You’ll Do as a Hospice Chaplain
  • Provide spiritual and emotional support to patients, families, and caregivers in alignment with their beliefs and values.
  • Offer non-denominational, inclusive care that respects all faiths, spiritual traditions, and cultural backgrounds.
  • Serve as an integral member of the interdisciplinary team, participating in patient care conferences and collaborating with nurses, social workers, aides, and medical staff.
  • Assess spiritual needs of patients and families and develop individualized spiritual care plans.
  • Conduct visits, prayers, blessings, and rituals as appropriate and requested by the patient or family.
  • Assist families during times of grief, loss, or transition, and provide bereavement support and counseling.
  • Build relationships with local faith leaders and community resources to support patients' and families' unique spiritual needs.
  • Maintain timely and accurate documentation of spiritual assessments, interventions, and outcomes.
About You

The candidate must meet one of the following educational requirements:

  • Bachelor’s degree in theology, religion, human services, counseling, psychology, or sociology (or as required by state-specific regulations)
  • A minimum of one unit of ACPE-accredited Clinical Pastoral Education (CPE)

Note: Degrees from accredited institutions are preferred. If a degree is from a non‑accredited college or university, AVPO approval is required prior to hire.
