Job summary

Ascend Hospice is hiring a Chaplain to provide spiritual care to patients and families, honoring diverse beliefs. This role collaborates with the hospice care team to ensure a continuum of compassionate end-of-life support.

Requirements include a Master's Degree in Divinity, 2–4 years of chaplain experience in end-of-life care, and at least 1 unit of CPE. Excellent communication and non-denominational care are essential.

Qualifications

  • 2–4 years of chaplain experience with end-of-life care.
  • At least 1 unit of CPE completed.
  • Ability to provide non-denominational spiritual care.
  • Excellent written and verbal communication skills.

Responsibilities

  • Provide pastoral or spiritual counseling aligned with patient/family beliefs.
  • Complete spiritual/pastoral assessments within 1 day of admission.
  • Develop personalized spiritual plan of care per regulations.
  • Offer discussions on beliefs to patients and families.
  • Coordinate pastoral services with the care team.
  • Provide bereavement services after death; assess 7–14 days post-loss.
  • Document care plan for bereaved survivors.
  • Pray with patients/families if desired.
  • Participate on the multidisciplinary Hospice team.

Now Hiring - Chaplain

The Chaplain will be responsible for, but not limited to:

  • Provides pastoral or spiritual counseling in keeping with patient/family beliefs, and in accordance with Ascend Hospice policies and procedures.
  • Completes spiritual/pastoral assessment as indicated and appropriate, completed within 1 day of admission.
  • Develops an individualized spiritual/pastoral plan of care, according to regulations.
  • Offers patients and families of different philosophies and religious beliefs opportunities to discuss and share their thoughts, feeling, beliefs and values.
  • Responds to hospice patient/family needs as requested.
  • Coordinates and integrates pastoral services with other patient/family care services in order to ensure a continuum of care, including continuity of care following death or discharge.
  • Prays with families and patients, if and when desired by the patient and/or family unit.
  • Effectively participates as a member of the multidisciplinary Hospice team, providing quality Hospice care to the patient and his/her family.
  • Provides bereavement services, in accordance with Ascend Hospice policies and procedures. Initiates bereavement assessment between 7 and 14 days after the death of the patient.
  • Documents a plan of care for the bereaved survivors, based on the IDT's assessment.
