Job summary

Visiting Nurse Home Care & Hospice is seeking a compassionate Hospice Chaplain to accompany patients and families through serious illness and end-of-life care, providing spiritual support, reflection, and comfort across home and hospice settings.

You will work with an interdisciplinary team, coordinate rituals, offer grief support, and document visits in compliance with regulations; requires seminary degree and CPE endorsement.

Qualifications

  • Graduate of seminary or theology school (BD/MDiv or equivalent).
  • Experience with death and dying and with individuals, families, caregivers.
  • CPE units or endorsement from a recognized faith body preferred.

Responsibilities

  • Provide direct spiritual & emotional support in homes, hospice facilities, or care settings.
  • Collaborate with the inter-disciplinary care team to integrate spiritual care.
  • Offer grief and bereavement support to families.
  • Coordinate religious rituals, sacraments, or ceremonies as requested.
  • Maintain accurate documentation of visits, assessments, and interventions.

As our Hospice Chaplain, you will be a steady, compassionate presence for patients and families navigating serious illness and end-of-life care. You’ll create safe spaces for reflection, meaning-making, and emotional healing—whether that’s through prayer, conversation, ritual, or simply listening. Your work will help patients feel seen, heard, and supported, and will bring comfort to families during some of their most challenging days.

Responsibilities
  • Provide direct spiritual and emotional support to patients and families in their homes, hospice facilities, or other care settings—respecting all faiths, traditions, and worldviews.
  • Collaborate with the interdisciplinary care team (nurses, social workers, aides, volunteers) to integrate spiritual care into each patient’s plan of care.
  • Offer grief and bereavement support to families before and after a patient’s passing.
  • Facilitate or coordinate religious rituals, sacraments, or ceremonies as requested by patients or families.
  • Maintain accurate, timely documentation of visits, assessments, and interventions in compliance with regulatory standards.
  • Provide spiritual support and self-care resources to staff and volunteers.
  • Build and maintain relationships with local faith leaders and community resources to support diverse spiritual needs.
Qualifications
  • Graduate of accredited seminary or school of theology (BD, MDiv or equivalent theology degree). Experience working with death and dying, individual/family/caregiver.
  • Certification or endorsement from a recognized faith body; Clinical Pastoral Education (CPE) units preferred.
  • Experience providing spiritual care in hospice, palliative care, or healthcare settings.
  • Deep empathy, active listening skills, and the ability to support people of all beliefs and backgrounds.
  • Strong communication skills and comfort working in patients’ homes and varied environments.
  • Ability to work as a member of the interdisciplinary team.
  • Knowledge and commitment to hospice philosophy of care.
  • A valid driver’s license and reliable transportation for home visits.
