Job summary

Extended Home Care in New York City is seeking a Speech-Language Pathologist in Clinical Fellowship Year (CFY) to perform home visits under supervision. You will assess, plan, and deliver therapy aligned with physician orders and agency policies.

Qualified candidates hold a Master’s in Speech-Language Pathology, NY licensure, and ASHA CCC; CFY experience preferred, with opportunities for growth within a supportive team.

Qualifications

• Must hold a master’s degree in Speech-Language Pathology and ASHA CCC.
• Licensed to practice as an SLP in New York state.
• CFY position with supervised clinical experience preferred.

Responsibilities

• Assess patients’ speech and language abilities and deficits.
• Prepare initial assessments and perform periodic reassessments.
• Develop and document a plan of care with measurable goals.
• Provide rehabilitative services to meet treatment goals.
• Document treatment types and patient progress in notes.
• Collaborate with health care team to coordinate care.
• Participate in conferences and discharge summaries as needed.
