Job summary

21st Century Home Health Services is seeking a Speech-Language Pathology CFY to provide speech and language therapy to patients in a home health setting. The role targets recent graduates completing CFY and involves assessment, diagnosis, and development of individualized treatment plans under supervision of a licensed SLP.

Qualifications

• Master’s degree in Speech-Language Pathology from an accredited program.
• Temporary CA state license or CFY certification to practice as a Speech Therapist.
• Strong communication, interpersonal, and organizational skills; ability to work independently.

Responsibilities

• Conduct comprehensive assessments of speech, language, voice, and swallowing functions in patients.
• Develop and implement individualized treatment plans based on assessment results.
• Provide therapy for speech, language, voice disorders, and swallowing difficulties.
• Collaborate with patients, families, and healthcare professionals to gather relevant information.
• Maintain accurate records of assessments, treatment plans, and progress; prepare reports as needed.
