Job summary

Tender Touch Rehab Services LLC seeks a compassionate Certified Physical Therapist Assistant to join our Healthcare & Medical Services team. This on-site role provides individualized therapy to seniors in skilled nursing and long-term care settings, implementing PT plans to improve mobility, safety, and quality of life.

You will work with an interdisciplinary team, perform therapeutic exercises, gait and balance training, and document in EMR while educating patients and caregivers.

Qualifications

• Certified Physical Therapist Assistant with CPR certification.
• Experience in geriatric therapy is a plus.
• Ability to document accurately in EMR systems.

Responsibilities

• Implement PT treatment plans to improve mobility, function and safety.
• Provide therapeutic exercise, gait and balance training, and functional activities.
• Educate patients and caregivers on care plans and disease management.
