Qualifications

• Peer Support Specialist Certification is required.
• BLS certification is required.

Responsibilities

• Ensures all actions, job performance, personal conduct and communications represent the organization in a highly professional manner.
• Conducts outreach services to individuals needing a medical home and/or with chronic disease.
• Assists patients in gaining access to and navigating a primary health care medical home and other community-based services; facilitates communication with the treatment team to achieve goals.
• Conducts eligibility determinations and enrollments for uninsured or underinsured patients; assists enrolling in the appropriate provider network.
• Provides recovery education to service recipients across all phases of recovery.
• Assist patients in creating individual wellness plans and developing goals with supports and resources.
