Job summary

ACMH is seeking a Certified Peer Specialist to join the Critical Time Intervention (CTI) team in Manhattan. You will provide supportive counseling and time-limited care coordination for high-risk clients, promoting coping skills and linking to resources.

Responsibilities include intake outreach, client engagement, and collaboration with the care team to improve health outcomes. Must possess NYS certification and a High School diploma; salary around $54,738 with generous benefits.

Qualifications

  • Lived experience of using mental health, substance use, or homeless services.
  • HS diploma or GED required; NYS Certified Peer Specialist credential required.
  • Good written and verbal English communication skills.
  • Basic computer and data entry skills.

Responsibilities

  • Engage clients at hospital bedside to provide program overview and intake assessments.
  • Develop rapport to help clients improve health and wellness.
  • Participate in weekly team meetings and provide updates on referrals.
  • Develop WRAP and Relapse Prevention Plans to manage triggers and cravings.
  • Educate clients on self-help techniques and coping strategies.
  • Provide transportation training and escorts to appointments.
  • Assist clients in building community support systems.
  • Document interventions and contacts in the EHR per standards.
  • Conduct home visits to assess living conditions and needs.

Lived experience as a consumer of mental health, substance use, and/or homeless services is required

Position: Certified Peer Specialist, Critical Time Intervention (CTI)

Function: Serves as a member of the CTI Team, providing supportive counseling and time-limited intensive care coordination for a caseload of high-risk clients. Responsibilities include promoting the development of skills for coping with and managing behavioral health symptoms, providing linkage to resources, and engaging individuals in skill building.

Reports to: Team Leader, Critical Time Intervention

Location: Manhattan

Schedule: Mondays-Fridays (9:00am-5:00pm)

Tasks
  • Responsible for engaging clients at hospital bedside to provide program overview and conduct intake assessments.
  • Develops rapport with clients in order to engage them in improving their health and wellness.
  • Participates in weekly team meetings to discuss new referrals and provide updates on enrolled clients.
  • Develops WRAP and Relapse Prevention Plans to help clients understand their patterns and triggers for use and identify interventions to deal with triggers and cravings and identify supports.
  • Acts as a role model to educate client about self-help techniques and self-help group processes, teaches effective coping strategies and teaches symptom management skills
  • Provides transportation training and escorts to appointments.
  • Assists clients in the development of community support systems and networks
  • Provides hands-on assistance with activities of daily living
  • Provides training in self-advocacy.
  • Supports client goals and serves as an advocate on client’s behalf
  • Identifies, facilitates and secures access to needed healthcare, social services benefits, and community resources. Works in collaboration with other care team members and care providers, including behavioral health, disease care management, home care, social work and community-based organizations, to help client achieve optimal health outcomes.
  • Embraces the team model by collaborating with members of the team and providing support as needed.
  • Utilizes evidenced based practices, such as motivational interviewing, to empower clients to grow and attain goals.
  • Implements tasks outlined on the care plan and ensures follow up and continuity of care between client interactions.
  • Documents all interventions and attempted contacts in the EHR in accordance with program standards.
  • Assesses domiciled client’s living conditions by conducting home visits.
  • Attends in-service training as requested.
  • Duties as assigned by supervisor.
Requirements

Lived experience receiving mental health services and a commitment to assist others in attaining recovery and wellness goals. High School diploma or G.E.D. Good written and verbal communication skills in English. Basic computer and data entry skills. Must be a Certified Peer Specialist with New York State.

Salary: $54,738 plus generous benefits
