Highland Rivers Behavioral Health seeks a Certified Peer Specialist to bring the lived experience to our team, engage individuals, and connect them with community resources and discharge planning.

You will document activities in the electronic health record, support recovery, and collaborate with the treatment team to promote recovery principles and client advocacy. Sharing one’s journey to inspire others may be part of the role.

Qualifications

• Lived experience with recovery and willingness to share experiences to inspire others.

Responsabilités

• Facilitate groups and structured activities focusing on self-advocacy and wellness, documenting outcomes in the health record.
• Identify client strengths and barriers to recovery.
• Provide follow-up calls for post-discharge referrals and ongoing services.
• Assist with discharge planning to ensure continuity of care.
• Orient individuals to the unit on entry into services.
• Link clients to internal programs and community resources with warm transfers when possible.
• Maintain up-to-date resource lists for community partners.
• Document in accordance with agency policies and complete required trainings/workshops.
• Participate in treatment team meetings, educating staff on recovery principles.
• Other duties as assigned.
