Job summary

UNM Hospitals seeks a Certified Peer Specialist to coordinate and provide services to individuals and families for rehabilitation, recovery, and resiliency. You will identify strengths and barriers, support crisis situations, and help develop recovery plans across age groups.

Responsibilities include promoting patient safety, coordinating with a multidisciplinary team, linking clients to community resources, and documenting encounters and progress.

Qualifications

• Essential: 1 year directly related experience
• Bilingual English/Keres, Tewa, Tiwa, Towa, Zuni, or Navajo
• CPR for Healthcare/BLS Prov or Prof Rescuers within 30 days
• Certified Peer Specialist recognized in State of NM
• Valid New Mexico Driver's License
• UNM Vehicle Operator's Permit within 60 days

Responsibilities

• PATIENT SAFETY 1 - Follow patient safety-related policies, procedures and protocols
• PATIENT SAFETY 2 - Proactively improve patient safety by questioning policies
• PATIENT SAFETY 3 - Identify and report unsafe conditions
• PATIENT SAFETY 4 - Report safety concerns and near misses
• PATIENT SAFETY 5 - Encourage patient participation in care
• NAVIGATION/ PATIENT ADVOCACY - Access community resources and services
• COLLABORATION - Work with multidisciplinary team to meet goals
• LIFE SKILLS - Develop daily living and community functioning skills
• WELLNESS - Provide information about conditions and medications
• SERVICE PLANS - Develop and coordinate service plans and crisis plans
• RESOURCE COORD - Help access rehabilitative and medical services
• SUPPORT SERVICES - Build natural supports in various environments
• ASSESSMENT - Identify needs and monitor progress
• CRISIS INTERVENTION - Assess and intervene in crisis situations
• TRACKING - Monitor service use and maintain client contact
• DOCUMENTATION - Document encounters and billing when appropriate
• STATISTICS - Report program metrics
