Job summary

Evolve Therapy Services, LLC is seeking a Certified Occupational Therapy Assistant to join our in-house team at a Skilled Nursing Facility in Fostoria, OH. You will implement planned therapy services and collaborate with a multidisciplinary team to deliver resident-centered care.

Qualifications

• Associate's degree in Occupational Therapy.
• Active OTA licensure in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification.

Responsibilities

• Screen residents to identify best OT interventions per scope of practice.
• Carry out treatment plans; follow short- and long-term goals with supervising therapist.
• Communicate evaluation results, plans, and recommendations to residents and families.
• Set individual therapy schedules with team members.
• Deliver therapy per plan and ensure proper documentation and billing.
• Perform other duties as required to effectively treat patients.
• Comply with CMS documentation standards daily with point-of-care notes.

Education
